問診表ご入力のお願い

外来受診の際、ネット予約の最後に問診票の入力欄が表示されます。あらかじめ症状や経過をご入力いただくと、診療がスムーズに進みます。問診表を入力された場合は、ご来院時に受付スタッフへお知らせください。診療前に医師が確認いたしますので、ご協力よろしくお願いいたします。

※帯状疱疹ワクチン（シングリックス）の助成での接種を受ける方は、ＨＰの問診票ではなく、品川区の予診票が必要になります。
あらかじめお手元に準備してからのご予約をお願いします。
